NOTE- This form is for team managers only.
The standard sizes issued for each team are listed below. If these sizes suit your team, you do not need to complete this form.
If we do not receive a size request within one week of your team registration, your team will receive the standard kit allocation for your age division.
We do not take colour requests.

Girls
•6/ 7 Girls: Size 6 × 4, Size 8 × 6
• 8/9 Girls: Size 8 × 4, Size 10 × 4, Size 12 × 2
• 10 Girls: Size 10 × 3, Size 12 × 6, Size 14 × 1
* 11 Girls: Size 10 × 2, Size 12 × 6, Size 14 × 3
• 12/13 Girls: Size 12 x 2, Size 14 × 6, Size 16 × 3
* 14 Girls: Size 12 x 2, Size 14 x 8, size 16 x 1
• 15 Girls: Size 16 × 4, Size S × 7,
Boys
• 6/7 Boys: Size 6 × 2, Size 8 × 5, Size 10 × 3
• 8/9 Boys: Size 10 × 6, Size 12 × 3, Size 14 x 1
* 10 boys: Size 10 x 5, Size 12 x 4, size 14 x 1
• 11/12: Boys: Size 12 × 3, Size 14 × 6, Size 16 × 2
• 13/14: Boys: Size 14 × 2, Size 16 × 2, Size S × 6, Size M × 1
